The drop-off form

This is the form a shop using Repairy prints and hands to a customer at drop-off. The customer fills in the details the repair needs. The text-updates box sits apart from those fields, is unticked until the customer ticks it, and the repair goes ahead either way. The same form is online at repairy.ai/opt-in.
